2. Complete the Application Form
Please note that completion of the application form will automatically place you on a waiting list to be processed.
Preschool staff will be in contact with you to arrange an Orientation Appointment.
What You'll Need to Apply
Before starting the application, please have the following documents ready:
- Birth Certificate
- Current ACIR Immunisation History Statement (available from Medicare)
- This must be current and up to date.
- Must be downloaded within the previous 2 months.
- Health Care Card (if applicable)
